Greenlife Healthcare Staffing is hiring a Licensed Master Social Worker (LMSW) to serve as a Nursing Home Surveyor / Complaint Investigator in New Rochelle, NY. In this rewarding role, you will ensure nursing home residents receive safe, compliant, and dignified care by assessing facilities for regulatory adherence.

Key Responsibilities:
Conduct on-site surveys and investigations of nursing homes, including abuse and neglect allegations
Draft Statements of Deficiencies (SOD) in line with Principles of Documentation
Evaluate compliance with Plans of Correction
Enter findings into federal and state reporting systems
Participate in state oversight during Immediate Jeopardy events
Attend regulatory meetings and provide testimony in administrative hearings
Adhere to DOH schedules and policies
Travel to assigned facilities according to a set schedule

What You'll Bring:
Licensure: NYS Licensed Master Social Worker (LMSW) - Required

Experience:
Minimum 2 years of clinical experience in a medical facility (preferably nursing home)
Prior experience in surveillance or regulatory investigations strongly preferred
Certification: SMQT Certification preferred (or willingness to obtain within 12 months)
Skills: Meticulous attention to detail, strong written communication, professionalism, and ability to work independently
